I'd go with:
one .22 lr/.410 shotgun survival combo
one 10 lb brick of ammo
an axe
magnesium
one kevlar gill net.

And to the asker, 10 lbs of ammo is a lot of ammo. If you only shoot at animals, that'd last you for YEARS. You can smoke meat to preserve it if you shoot a larger animal. Other than that, after you're out of ammo, you become a vegetarian or catch fish.

I'm with Happy*, except I would add 3 bottles of good single malt and 10 pounds of prime beef, to bring me up to the 5 items. Under those parameters, the rest of your life won't take long, might as well enjoy it.
